Cancer Chemotherapy by Continuous Intraarterial Infusion in Inoperable Cancer Patients



Abstract
Continuous intra-arterial infusion chemotherapy in 10 patients with liver cancer, maxillary cancer, and rectal cancer at Dept. of Surgery, S. N. U. H. was reviewed.
Catheter was inserted; in cases of maxillary cancer, into the left external carotid artery and the common carotid artery through the left superficial temporal artery; in cases of liver cancer, into the hepatic artery through the right gastroepiploic artery or gastroduodenal artery; in the case of
recurrent rectal cancer, into the branch of the inferior mesenteric artery.
1) Methotrexate group. (5 cases)
One case was maxillary cancer and 4 were liver cancer (primary 3, metastatic 1). All cases had no previous treatment. Methotrexate 25 mg. per 24 hours in 1, 000 ml. of 5% D/W was given for 3-8 days. Citrovorum Factor.(C.F.) was given intramuscularly 3 mg. every 6-8 hours (9-12 mg. per 24 hours) for 3-10 days, except in one case. In all cases, the size of tumor mass was notably reduced and the subjective symptoms were slightly improved. Duration of response was within 5 weeks and follow up study revealed 37patients expired in 5 months after treatment. As the toxic effects of Methotrexate, various gastrointestinal disturbances, such as, stomatitis, nausea and diarrhea occurred in 4 cases, leucopenia in 4 cases, nasal bleeding with thrombocytopenia in 2 cases and chemical hepatitis in 2 cases. Complicatitis in 2 cases. Complications developed in 2 cases; clot in the catheter in 1 case and pneumonia and cellulitis in 1 case.
2) 5-Fluorouracil group. (5 cases)
One case was maxillary cancer, 3 cases were liver cancer. (primary) and 1 case was locally recurrent rectal cancer. 5-FU 250-450 mg. per 24 hours was infused for 3-26 days. In case of maxillary cancer, cancer cells were completely disappeared by pathohistologic study; in 3 cases of liver cancer, the size of tumor mass was notably reduced and the subjective and objective symptoms were improved. In case of recurrent rectal cancer, subjective symptoms were slightly improved. As the toxic effects of 5-FU, nausea (1 case), diarrhea (2 cases) and leucopenia (1 case) were occurred. Complications developed in 2 cases; clot in the catheter in 1 case and leakage of infusate in 1 case.
